Grade 9 Immunization Forms – Due Back Nov.30

If any grade 9 students were absent on Monday for HomeRoom, please come to the office to pick up your Immunization form.

Immunization consent packages for grade 9 students were sent home from the school on November 21st with report cards. We hope that parents/guardians will discuss the information in the consent package with your child and include them in the decision to provide consent for immunization. Please return the completed consent form to the school office in a sealed envelope as soon as possible or by November 30, 2022.  Students in grade 9 will be offered immunizations in school-based clinics in February 2023.

Grade 9 students can make an informed decision about immunization regardless of their parent/guardian/representative decision to consent or refuse immunization.  For more information visit HealthLinkBC: The Infants Act, Mature Minor Consent and Immunization.

If you have any questions, please contact the local Health Unit at 250-544-2400.
